You can schedule a sick visit here if you have created an account.

For first time users, an invitation to the portal has to be sent to your email so please call the office where you would like to be seen:

These appointments can only be scheduled for any type of sick visit. Physical or Follow up appointments will need to be submitted through "Request An Appointment" or you may call the office to schedule.
